Unmetabolized methadone and its metabolites are excreted in urine to a variable degree. A common term for the type of treatment at a methadone clinic is "replacement therapy".

Tolerance to the different physiological effects of methadone varies; tolerance to analgesic properties may or may not develop quickly, but tolerance to euphoria usually develops rapidly, whereas tolerance to constipation, sedation, and respiratory depression develops slowly (if ever).[35] Driving[edit] Methadone treatment may impair driving ability.[36] Drug abusers had significantly more involvement in serious crashes than non-abusers in a study by the University of Queensland. It can be used as a pain reliever or as part of drug addiction detoxification and maintenance programs. The length of time a person remains in treatment depends on a number of factors. It can also cause side effects that impair your thinking and reactions.
Talk to your doctor about the risks of taking methadone for the condition for which you are being treated. Once a person has a strong footing in recovery, their doctor may slowly the dose of methadone over time to wean them off the substance. People taking methadone should contact their doctor if they: have difficulty breathing; become very drowsy and breathing slows down; have little movement of the chest with breathing; have a fast or slowed heartbeat; feel faint, extremely dizzy, confused, have irregular heartbeats or any other symptoms that are not typical. Respiratory depression is the chief hazard associated with methadone hydrochloride administration.
Physicians find that education is a productive way to impress the dangers of methadone upon their patients and their loved ones. Due to its activity at the NMDA receptor, it may be more effective against neuropathic pain; for the same reason, tolerance to the analgesic effects may be less than that of other opioids.[18][19] People with long-term pain will sometimes have to perform so-called opioid rotation.[20] Opioid rotation involves switching from one opioid to another, usually at intervals of between a few weeks, or more commonly, several months. If you are taking methadone to control pain, your pain may return before it is time for your next dose of methadone.
Methadone is available in traditional pill, sublingual tablet, and two different formulations designed for the person to drink. Although this medication is an opioid itself, it has been used to treat people with an addiction to heroin (or another type of opioid) for more than 45 years. Some patients will be on methadone for the rest of their lives, which generates criticism regarding the effectiveness of the clinics. However, if you cannot access a doctor for some reason, then your initial doses of Methadone should be very, very small. However, children born to women treated with methadone during pregnancy have been shown to demonstrate mild but persistent deficits in performance on psychometric and behavioral tests. Although this medication is an opioid itself, it has been used to treat people with an addiction to heroin (or another type of opioid) for more than 45 years. It is also possible to do this on your own, but not recomended because opiate addiction is exstreamly difficult to control by yourself.
